Tooth enamel is about 90% hydroxyapatite. This powder is HAp made by a patented synthesis route and combined with zinc oxide and collagen, milled to an average particle size below 0.15 µm so that the surface area available to fill micro-defects in the enamel surface is high. It is sold as an ingredient to oral-care formulators, not as a finished toothpaste.

No. It is the active powder, sold by the kilogram to formulators. You add 0.5–2% into your own base.
Because the mechanism depends on surface area. Below 0.15 µm the powder presents far more surface per gram, which is what allows it to sit in the micro-defects on an enamel surface rather than simply being rinsed away.
Zinc oxide contributes to the plaque and oral-odour side of the formulation, alongside the HAp's mineral role.
